suppliers for opportunities that include solocitation for bids, equipment purchases and other areas. Should your company

to identify the certification agency when completing this form. A list of recognized certification agencies may be downloaded
by selecting the "TLC Recognized Certification Agencies" list. We encourage all qualified applicants to participate.
In addition, we encourage our existing suppliers to make a concerted effort when sourcing equipment to utilize qualified MWDVBE
certified companies.
It is our desired goal to achieve the following
percentages of aggregate estimated annual purchases. 15% annual MBE participation; 5% annual WBE participation; and 1.5% annual
DVBE participation.
